  • Over 300,000 lbs.
    • $350.00 + $3 per 1000 lbs.
  • Over 500,000 lbs.
    • $350.00 + $3 per 1000 lbs. + Additional $.05 per 1000 lbs. per mile (based on
      total GVW)
